Long-term conditions
Long-term conditions are conditions that cannot be cured but can be controlled by medication and other therapies. For example, heart disease, diabetes and respiratory disease.
The PCT has a Long-term Conditions team, which aims to improve the health and quality of life of older people and people with long-term conditions. They work closely with health and social care colleagues, voluntary organisations, patients and the public to improve care and services for people with long-term conditions.
In this section of the you can find out about the services and support available for people with a wide range of long-term conditions, including Chronic Obstructive Pulmonary Disease (COPD) and other respiratory conditions, diabetes, heart disease, cancer and other life-threatening diseases and stroke.
You can also find out about the role of our community matrons and our Expert Patient Programme, which help people with long-term conditions to take control of their lives. And there is advice on avoiding falls, which are one of the biggest causes of injury and ill-health in older people.
