If you get a letter from NHS Wakefield District and have any questions about the new Summary Care Records service or how to opt out please ring our Patient Advice and Liaison Service (PALS) on 0845 6024832.

Advisers are available Monday to Friday from 10.00am to 4.00pm. You can also email the service on pals@wdpct.nhs.uk with your enquiry and an adviser will get back to you as soon as possible. Please note; lines may be busy at peak times but we will try to answer your call as quickly as we can.

Remember, if you receive a letter and are happy with the changes, you do not need to do anything as your Summary Care Record will be created for you.
